What are discharge expectations?

The rehabilitation team will decide appropriate recommendations and equipment that is needed upon discharge.  All appointments and equipment needed will be arranged prior to discharge in consultation with the doctor and the rehabilitation director.

Each discharge plan will be determined based on a combination of functional abilities and available family and community supports. Some people can return to their home environment with a care plan that may include professional services such as nursing, physical, speech, and occupational therapy, etc. These services are provided in a less intensive manner, over an extended length of time.

The rehabilitation team will discuss the most appropriate options with each patient and their family. Our goal is to construct the best possible plan of care at discharge for each patient.
